How to Request Medical Records

Please request records through Chart Request by signing up for an account and completing the instructions.

Fees

All records sent electronically should be free of charge for patients and healthcare providers.  If you are requesting paper records, there may be a fee for postage.  Third-party entities requesting records may be charged a fee to fulfill the request.

Request Turnaround Time

Medical Record Requests may take approximately 5-7 business days to be completed. If you have not received your records in that time frame or if you have any other questions, please contact us at 208-587-3988.
